Cinquante patientes ont bénéficié de prélèvements macrobiopsiques en raison de l'existence de foyers de microcalcifications mammaires classés ACR4 ou ACR5. La procédure a été réalisée soit en 11G (mammotome® Breast Care) soit en 10G (Vacora®, Bard). Au décours de la procédure, un clip a été mis en place : Twist Marker® (Bard) : anneau en nitinol, se fixant à la paroi de la cavité lors de son déploiement. Un contrôle immédiat stéréotaxique et un contrôle mammographique à 8 jours ont été réalisés.

Résultats

La procédure est de réalisation aisée, le marqueur peut s'utiliser avec les deux modes de macrobiopsies en raison de l'existence de deux versions, droite et courbe. Le mécanisme de déploiement est un retour à la forme initiale du marqueur étiré dans son mandrin, entraînant un ancrage du clip dans le tissu de la cavité. Le taux de déplacement apparaît comparable aux autres techniques, de l'ordre de 15 %.

Conclusion

Il s'agit d'un clip fiable qui permet, en cas d'exérèse complète de la cible, un repèrage satisfaisant en cas de reprise chirurgicale potentielle.
